Qualified Developmental Disabilities Professional
Responsible for coordinating Individual Service Plans (ISP) every year to assess individual’s progress, ensure participation in valued activities of individual’s choice and establish training programs for daily living skills.
Ensure completion of all paperwork in a timely manner including but not limited to quarterly reviews, ISP’s, assessments, data forms, various medication sheets, behavior charts, and quarterly QA reports. Coordinate with other agencies and departments, as well as parents and advocates on individual’s behalf to solve problems and provide assistance.
Attend the individual’s ISP meetings or designate appropriate QDDP staff to attend individuals ISP
meetings two times per year.
Duties And Responsibilities
- Responsible for all programmatic documentation – assessments, Individual Service Plans (ISP), behavioral plans, goal tracking, periodic reviews and quality assurance reviews.
- Schedule and facilitate Person Centered Meetings for Gateway to Learning caseload
- Communicate professionally with members of the person-centered team including participant, guardians, family members, staff, ISC, community members, etc.
- Manage data collection for plan development
- Work with team to develop implementation strategies
- Maintain data tracking records and all file documentation, including monthly Q notes
Requirements
- At least one year of documented experience working directly with individuals who have developmental disabilities
- Is one of the following: a doctor of medicine or osteopathy; a registered nurse; a provider holding at least a bachelor's degree in a human service field including sociology, social work, special education, rehabilitation engineering, counseling, or psychology
- Must have a Valid Virginia’s Driver’s License and copy of current vehicle insurance
- Criminal Background Investigation and a registry by the Department of Social Services
- Must go through New Employee Orientation which will require them to complete certain assigned training (CPR/First Aid, TOVA, any Victory’s Village training, etc.)
Other Requirements
- Must have a strong understanding of Person-Centered Planning for Individuals with Developmental Disabilities.
- Must understand key adult educational techniques related to best practices for Individuals with a Developmental Disability.
- Must have excellent organizational skills, proficient on Word, Excel and other databased software.
- Must have excellent written and oral communication skills.
