SUPPORTIVE IN-HOME SERVICES
Victory’s Village provides Supportive In-Home services to Individuals diagnosed with a developmental disability.
These services take place in the Individual’s home, family home, or community settings.
Through Supportive In-Home services, Victory's Village provides Individuals with skill-building and routine support to complete Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs), promote independence, maintain a healthy routine, and to live the best version of their lives.
Thorough assessment and diagnosis of comprehensive needs
Counseling for individuals and families
Round-the-clock crisis support available seven days a week
Interventions for behavioral modification and training in parental skills
Aid in accessing and navigating community resources
Who's Elgible
Children or adolescents facing the potential of out-of-home placement
Children or adolescents in the midst of transitioning back to their homes from an out-of-home placement

Pre Screening
Please contact our office to receive a pre screening via telephone.
Intake
If it is determined that your child or family member is eligible for Supportive In-Home Services through Victory’s Village.
An intake meeting will take place with the Individual and their Village (their family and or legal guardian, and other supporting parties) to complete their Person-Centered Plan.
During this time The Village will work together to determine and collaborate on a personalized plan that best fits the needs of the Individual.
Services
After completion of the Person Centered Planning, an Individual will then begin to receive comprehensive, face to face services from a Direct Support Professional (DSP) in his/her home or family member’s home.
